- Plain and simple, you are going to simply add a custom field to the form you are using for your opt-in, or create a new form and call it Contact Me and use that one. Either way, the end result is the same. Here are the instructions on how to create a form.
- Then turn on notifications to yourself or a team member and that way you'll know when someone submits the form. If you have a heavy influx of opt-ins, then you'll probably want to create a separate form Contact Me and use that or your inbox will quickly be overrun by basic submissions.
